What Are Housing Stabilization Services?
Housing Stabilization Services (HSS) is a Minnesota Medical Assistance (MA) benefit that helps individuals with disabilities, seniors, and those experiencing housing instability to find and maintain safe, stable housing. These services are especially helpful for those without case managers, those facing eviction, or those living in unsafe or transitional housing situations.
Our team works closely with clients to understand their goals, navigate the housing system, and remain housed for the long term.
We Support People Who Are…
Enrolled in Medical Assistance (MA)
Diagnosed with a disability (physical, mental, or cognitive)
Living in unstable, or temporary housing situations
Without a targeted case manager or care coordinator

Housing Consultation
When: Before services begin — for those without a waiver or case manager
What We Do:
Develop a housing-focused, person-centered plan
Help clients choose their transition/sustaining service provider
Coordinate with other care professionals
Offer resources related to non-housing needs
This is Step 1 in getting approved for HSS.
Housing Transition
When: You're searching for housing or planning a move
What We Do:
Assist in identifying preferred housing
Help with the application process
Develop housing budgets and review leasing terms
Provide guidance and housing stability planning
This helps you move from homelessness or unstable housing into a home.
Housing Sustaining
When: After you move into your home
What We Do:
Educate on tenant rights and lease responsibilities
Provide coaching on landlord relationships
Help with conflict resolution
Support long-term housing stability and independence
We stay with you to help maintain your housing.
